Payment, Refund & Cancellation

Effective Date: 21st December 2021

Your satisfaction with the Services provided to you by Epanafero Ltd. (“Epanafero”, “My Heart Wellness”, “Company”, “we”, “us”) matters greatly to our team.  We invest considerable time, effort, preparation and care into creating and providing our Services so that we can meet your needs and expectations.

As we tailor our Services according to our participants, our payment, cancellation and refund terms are strictly applied and enforced.

Each Service has its own specific payment, cancellation and refund terms, as detailed in this Payment, Cancellation and Refund Policy.  Please read the terms below carefully and make sure that you understand the terms that apply to your desired Service(s) before placing your order.

During the ordering process, you will see reference to the specific payment, cancellation and refund terms that apply to the Service(s) on your order page. You may be asked to read and confirm your acceptance these terms before completing your order.  The terms may also be included in your order confirmation email remitted by us.

This Payment, Cancellation and Refund Policy is subject to and incorporated within our Terms and Conditions.  Please refer to our Terms and Conditions for details on how to change/cancel your order and the payment of any refund if applicable.

1. Heart Health & Wellness Membership

Our heart health & wellness membership (“Membership Service”) is billed on a subscription basis (“Subscription”).  Payment in full must be made in advance for you to access this Membership Service for the period you selected when purchasing your Subscription (1 month, 4 months, or 1 year).

Your recurring membership will remain active, accessible, and usable on an ongoing basis for as long as you remain current with your Subscription fees.  Failure to pay your Subscription fees may result in termination of your account and a loss of any/all data stored or entered therein.  You may be required to rejoin at higher rates.

We reserve the right to amend the Membership Service’s subscription pricing, payment methods, payment processes, membership features and these Terms at any time, and without prior notice.  All other changes will apply from the date that the amended or new Terms are posted on our website or are provided to you by email, whichever is earlier.

Your Membership Subscription cannot be transferred to another person or entity.

Payment

You will be billed in advance on a recurring and periodic basis (“Billing Cycle”). Billing cycles are set on a monthly, 4-monthly, or annual basis, depending on the subscription plan you selected when purchasing your Subscription.

A valid payment method (such as credit card) is required to process the payment for your Subscription. You shall provide us with accurate and complete billing information including full name, address, state, zip code, telephone number, and valid payment method information.  By submitting such payment information, you automatically authorize us to charge all Subscription fees incurred through your account to any such payment method.

Upon receiving your Subscription payment, we will provide you access to this Membership Service.  You will retain this access for the duration of your Subscription period, as follows:

  • A monthly subscription allows you access to the Membership Service for 30 days;
  • A 4-monthly subscription allows you access to the Membership Service for 120 days;
  • An annual subscription allows you access to the Membership Service for 365 days.

Your first Subscription payment will be payable on the date you purchase your Subscription plan via the Website.

At the end of each Billing Cycle, your Subscription will automatically renew under the exact same conditions unless you cancel it or we cancel it.  You will be charged each period on the same date that you signed up – this is your scheduled payment date.  For example, if you purchase a monthly plan on 10th February, your next payment will be on 10th March, then on 10th April etc.  Where this day falls on a non-work day, the payment may be initiated on the next working day.

It is your responsibility to ensure that there are sufficient cleared funds using your nominated payment method on each scheduled payment date in order to meet your Subscription payment.

Should automatic billing fail to occur for any reason – for example, your credit card payment declines on the scheduled payment date – we reserve the right to continue attempts to process your payment.  We will attempt to process the declined amount until successful, up to a maximum of retries.  We may also issue you with an electronic invoice indicating that you must proceed manually, within a certain deadline date, to make the full payment corresponding to the billing period indicated on the invoice. Failure to pay your subscription amount within this deadline date will result in your access to the Membership Service ceasing at the end of your current contracted Subscription period.

Should your credit card expire or be cancelled, it is your responsibility to provide us with your new credit card details (via your online Account management page or by contacting us at care@myheartwellness.com to determine how to update your credit card information) to ensure that your Subscription payments continue to be successfully made.

14-day Satisfaction Guarantee

Your satisfaction with our Membership Service is our primary concern.  We therefore offer a 14-day money-back Satisfaction Guarantee on all our subscription plans which gives you full access to our Membership Service during the Guarantee Period to assess your satisfaction with this Service in your first month of membership.

The Guarantee Period is 14 days starting on your Subscription commencement date (i.e. the date when we receive your first Subscription payment and provide you with access to our Membership Service) in your first month of membership.

To be eligible for this Satisfaction Guarantee, you must meet the following conditions:

  • You accessed the Membership site using your login credentials; AND
  • You attended at least one of the events scheduled for our Membership (such as course webinar, live Q&A, interview) OR watched at least one of our video course modules.

Should you meet these requirements and wish to cancel your Membership Service during the Guarantee Period, you can request a refund under this Guarantee.  To do so, please cancel your account via your online Account management page and let us know using the Membership Site’s chat feature or by email at care@myheartwellness.com.

Refunds will be made to the credit card or other payment method that you used to purchase your Membership Subscription.  We will process your refund within a maximum of two weeks from your request, and will confirm the same by email.

Note that this 14-day Satisfaction Guarantee applies only to your first Subscription purchase and cannot be used more than once by any individual.  Any cancellation request made outside of the Guarantee Period will be treated as a membership cancellation and subject to the terms outlined below.

If you’re really not getting what you need from our Membership Service, we want you to take advantage of this Satisfaction Guarantee and tell us how you think we can improve. We may ask you for 15 minutes of your time via a Zoom call so you can tell us where we’re going wrong and how we could be better.  This is not a sales tactic, nor will we ask you any difficult questions or guilt trip – we would simply appreciate your honest feedback to help us make this Service better!

Cancellations

You may cancel the automatic renewal of your Membership Service Subscription at any time, either through your online account management page or by contacting us by email at care@myheartwellness.com.  Should you or we cancel your Subscription renewal, your access to the Membership Service will cease at the end of your current contracted Subscription period.

You must submit your cancellation request at least 48 hours* prior to your next scheduled payment date to prevent your next payment being charged. We cannot guarantee your cancellation request will be processed in time if it occurs within 48 hours* of 12:00am EST on the billing date. The billing process is automated, but the cancellation process must be handled manually.

* NOTE: Special provision applies to cancellation requests near to major holidays.  You must cancel at least 3 working days before any of the following major holidays: Christmas Eve, Christmas Day, Boxing Day, New Year’s Eve, New Year’s Day, the Easter weekend (from Good Friday to Easter Monday, inclusive) plus any Bank Holidays observed in the US, Canada, or the UK.

It is entirely your responsibility to ensure that you cancel your Subscription in good time bearing in mind these cancellation processing times.

As soon as your cancellation request has been processed, you will no longer have access to this Membership Service (including all content and community resources) once your current contracted Subscription period ends. This means that you will not be able to log into the Membership site using your log-in credentials.  If you did not submit your cancellation request in time and the current month’s payment has already processed, we will not issue a refund for the current month or reinstate your membership until the next billing cycle.

Due to system limitations, we are unable to honor requests to cancel on a future date. If you wish to retain access to this Membership Service until your next billing date, it is your responsibility to request your cancellation at the appropriate time.

If you cancel and then decide to join our heart health & wellness membership again subsequently, you will be reinstated at the current membership rate, regardless of any special offers or discounts you received in the past.

Refunds

Due to the digital nature of this digital Membership Service and because you gain immediate access to the members’ area and all the content on our Wellness Membership site, your order cannot be “returned”.  You cannot place your order and then request (and be granted) a refund straight away.  For all subscription plans (monthly, 4-monthly, and annual), cancellations will apply to your next scheduled payment period.

Outside of the afore-mentioned 14-day guarantee window, the following refund terms apply to subscription fees already paid:

  • No partial (or prorated) refunds are given should you terminate a monthly subscription or a 4-mothly subscription before your renewal date; you will however retain access to the Membership Service until the end of your current contracted payment period.
  • Certain partial refund requests for annual subscriptions may be considered by us on a case-by-case basis and granted at sole discretion of the Company.  There will be a $150 charge to process such a refund to compensate for bank and administrative charges as well as the customer service, content delivery, coaching and other services provided to you during your subscription period.

No refund is available if you do not access or make use of your Membership Service during your subscription period, and nor is a refund available for any activities offered as part of your membership subscription that you miss.

No refund is available if you access or make use of your Membership Service and are dissatisfied with its presentation or content, with the exception of our afore-mentioned 14-day Satisfaction Guarantee.

We highly recommend that you ask questions to make sure our Membership Service is right for you before you purchase a Subscription.  You can send your questions to us by email at care@myheartwellness.com.

Free and Paid Trials

We may, at our sole discretion, offer a Subscription with a free or paid trial for a limited period of time (“Free Trial”, “Paid Trial”).

Free Trials: you may be required to enter your billing information to sign up for a Free Trial.  If you do enter your billing information when you sign up for the Free Trial, you will not be charged by us until the Free Trial has expired.  On the last day of your Free Trial period, unless you cancelled your Subscription, you will be automatically charged the applicable Subscription fees for the type of Subscription you have selected.

Paid Trials: you will be required to enter your billing information to sign up for a Paid Trial, and will be charged immediately the trial amount (e.g. $1).  On the last day of your Paid Trial period, unless you cancelled your Subscription, you will be automatically charged the applicable Subscription fees for the type of Subscription you have selected.

At any time and without notice, we reserve the right to (i) modify the terms and conditions of the Free Trial and Paid Trial offer, or (ii) cancel such Free Trial or Paid Trial offer.

Subscription Fee Changes

We may, at our sole discretion and at any time, modify the Subscription fees for the Subscriptions.  Any Subscription fee change will become effective at the end of your then-current Billing Cycle, which means that you will pay the new Subscription fee amount on your next subscription renewal date.

We will provide you with a reasonable prior notice of any change in Subscription fees to give you an opportunity to terminate your Subscription before such change becomes effective.

Your continued use of the Service after the Subscription fee change comes into effect constitutes your agreement to pay the new Subscription fee amount.

Your Access to this Membership Service

Access is for the lifetime of this Membership Service.  If for any reason, Epanafero Ltd. should dissolve or cease to exist, then your access to this Membership Service terminates with immediate effect.

We reserve the right to modify or discontinue materials, features, and scheduled events from this Membership Service without prior notice.  We also reserve the right to remove content from membership areas and discontinued.  Such material will become inaccessible at that point, even to Lifetime Access members.

You agree and understand that access to this Membership Service may at times be influenced and affected by third parties that we use to provide this Service (including web hosting, membership site plugins, etc.). You agree and understand that no breach of contract action may be initiated against us when there are reasonable delays in the access of the Membership Service.

Termination or Suspension of your Membership Account

We may terminate or suspend your Account immediately, without prior notice or liability, for any reason whatsoever, including without limitation if you breach any of our Company’s Terms.  Please read carefully our Terms & Conditions, which are available on our Website.

Upon termination, your right to use the Service will immediately cease.  If you wish to terminate your account, you may simply discontinue using the Service.

In the event of cancellation or termination of your Account, you are no longer authorized to access the part of the Site affected by such cancellation or termination. The restrictions imposed on you with respect to material downloaded from the Site, and the disclaimers and limitations of liabilities set forth in our Terms & Conditions, shall survive.

All provisions of the Terms which by their nature should survive termination shall survive termination, including, without limitation, ownership provisions, warranty disclaimers, indemnity and limitations of liability.

If Epanafero Ltd. Terminates this Membership Service

We reserve the right to terminate this Membership Service, and or access to certain of its features, with or without prior notice to you.  We will make reasonable efforts to provide you with notice but are not required to do so under these terms.

In the unlikely event that we need to terminate this Membership Service for any reason, we will offer you a partial refund only if you have an annual subscription plan.  Please note that we will not offer a partial refund if you have either a monthly subscription or a 4-mothly subscription.

If Epanafero Ltd. Cancels a Scheduled Membership Event

On rare occasions, due to unforeseen circumstances, we may need to cancel an event scheduled for our Membership such as a course webinar, live Q&A, interview etc.  We will notify you of such a cancellation with as much notice as possible and will reschedule.

2. Wellness Online Course

Payment

Payment in full must be made in advance for your Wellness Online Course.

Neither your Wellness Course nor any of your Course sessions may be transferred to another person.

Scheduling of Your Course Sessions

All Course sessions are scheduled in advance by us.

If You Are Late for a Scheduled Course Session

If you are late for a scheduled Course session for whatever reason, the session will run to the original agreed time.

If you fail to show within 30 minutes of the agreed start time of a Course group session, you will be considered a “no show” and you will forfeit your right to join the session even if it is still ongoing for the other Course participants.

My Heart Wellness will share a recording after each Course group session with all participants enrolled in the Course so you will be able to catch up with any session(s) that you miss.  If you have any questions about the material covered, you will also have an opportunity to speak with the My Heart Wellness team during the Course’s scheduled Office Hours.

Cancellations and Refunds

If You Cancel Your Wellness Course

  • If you cancel more than 30 days prior to start date of your Course: A cancellation fee (USD 100 per person) will be applied, and the balance which remains from the amount you have paid will be refunded to you OR held on account (at your election) for ONE (1) YEAR following the date of cancellation for your use towards a future course.
  • If you cancel between 14-30 days prior to start date of your Course: A cancellation fee (USD 200 per person) will be applied, and the balance which remains from the amount you have paid will be held on account for ONE (1) YEAR following the date of cancellation for your use towards a future course.
  • If you cancel fewer than 14 days prior to start date of your Course: The full amount you have paid will be forfeited.

No refund is available if you do not show up for your Course, and nor is a refund available for any Course sessions that you miss.

No refund is available if you attend your Course and are dissatisfied with its presentation or content.

Any monies held on account which are not used by you within the time frame specified above will be forfeited.

If Epanafero Ltd. Cancels Your Wellness Course

On rare occasions, due to unforeseen circumstances, we may need to cancel a wellness course.  We will notify our participants of such a cancellation a minimum of SEVEN (7) calendar days in advance of the course’s start date and will offer a full refund for the course OR the option to rebook another course within ONE (1) YEAR of the cancellation notice.

If Epanafero Ltd. Cancels a Scheduled Course Session

On rare occasions, due to unforeseen circumstances, we may need to cancel a scheduled Course session.  We will notify you of such a cancellation with as much notice as possible and will reschedule.

3. Wellness Coaching

3.1 Individual (1-1) Coaching

Payment

Payment in full must be made in advance for your Individual Wellness Coaching package.

Neither your Individual Wellness Coaching package nor any of your Coaching sessions may be transferred to another person.

Scheduling Your Coaching Sessions

All Coaching sessions are scheduled in advance by direct agreement between you and us.

All the Coaching sessions in your package must be used within 3 months of your order confirmation.  After that time, any unused Coaching session(s) will be deemed forfeited and shall be forfeited.

Cancellations and Refunds

We do not offer refunds for any of our Coaching programs or Coaching sessions.

If You Are Late for a Scheduled Coaching Session

If you are late for a scheduled session for whatever reason, the session will run to the original agreed time.  If, however, you fail to show within 20 minutes of the agreed start time, you will be considered a “no show” and you will forfeit your right to the session missed.  Such “no show” sessions will not be rescheduled.

If You Cancel a Scheduled Coaching Session

If for any reason you cannot attend a scheduled session, you are responsible for notifying us by phone or email and ensuring that we receive your communication.  The following apply:

  • Cancellations received by us at least 48 hours in advance of a scheduled session will be rescheduled with you.
  • Cancellations received by us less than 48 hours in advance of a scheduled session will not be rescheduled, and the session time will be considered forfeited.

If Epanafero Ltd. Cancels a Scheduled Coaching Session

On rare occasions, due to unforeseen circumstances, we may need to cancel a scheduled session.  We will notify you of such a cancellation with as much notice as possible and will reschedule with you.

3.2 Couple (2-1) Coaching

Payment

Payment in full must be made in advance for your Couple Wellness Coaching package.

Neither your Couple Wellness Coaching package nor any of your Coaching sessions may be transferred to another person.

Scheduling Your Coaching Sessions

All Coaching sessions are scheduled in advance by direct agreement between both coaching partners and us.

All the Coaching sessions in your package must be used within 3 months of your order confirmation.  After that time, any unused Coaching session(s) will be deemed forfeited and shall be forfeited.

Cancellations and Refunds

We do not offer refunds for any of our Coaching programs or Coaching sessions.

If You or Your Spouse Are Late for a Scheduled Coaching Session

If you or your spouse are late for a scheduled session for whatever reason, the session will run to the original agreed time.

If one of you fail to show within 30 minutes of the agreed start time, that person will be considered a “no show” and will forfeit his/her right to the session even if it is still ongoing for the other person.  Such “no show” sessions will not be rescheduled.

If You Cancel a Scheduled Coaching Session

If for any reason both you and your spouse cannot attend a scheduled Coaching session, you are responsible for notifying us by phone or email and ensuring that we receive your communication.  The following apply:

  • Cancellations received by us at least 48 hours in advance of a scheduled session will be rescheduled with you both.
  • Cancellations received by us less than 48 hours in advance of a scheduled session will not be rescheduled, and the session time will be deemed forfeited by you both.

If Epanafero Ltd. Cancels a Scheduled Coaching Session

On rare occasions, due to unforeseen circumstances, we may need to cancel a scheduled session.  We will notify you of such a cancellation with as much notice as possible and will reschedule with you both.

4. Wellness Monitoring

4.1 Individual Monitoring

Payment

Payment in full must be made in advance for your Individual Wellness Monitoring package.

Neither your Individual Wellness Monitoring package nor any of your Monitoring sessions may be transferred to another person.

Scheduling Your Monitoring Sessions

All Monitoring sessions are scheduled in advance by direct agreement between you and us.

All the Monitoring sessions in your package must be used within 3 months of your order confirmation.  After that time, any unused Monitoring session(s) will be deemed forfeited and shall be forfeited.

Cancellations and Refunds

We do not offer refunds for any of our Monitoring programs or Monitoring sessions.

If You Are Late for a Scheduled Monitoring Session

If you are late for a scheduled session for whatever reason, the session will run to the original agreed time.  If, however, you fail to show within 20 minutes of the agreed start time, you will be considered a “no show” and you will forfeit your right to the session missed.  Such “no show” sessions will not be rescheduled.

If You Cancel a Scheduled Monitoring Session

If for any reason you cannot attend a scheduled session, you are responsible for notifying us by phone or email and ensuring that we receive your communication.  The following apply:

  • Cancellations received by us at least 48 hours in advance of a scheduled session will be rescheduled with you.
  • Cancellations received by us less than 48 hours in advance of a scheduled session will not be rescheduled, and the session time will be considered forfeited.

If Epanafero Ltd. Cancels a Scheduled Monitoring Session

On rare occasions, due to unforeseen circumstances, we may need to cancel a scheduled session.  We will notify you of such a cancellation with as much notice as possible and will reschedule with you.

4.2 Couple Monitoring

Payment

Payment in full must be made in advance for your Couple Wellness Monitoring package.

Neither your Couple Wellness Monitoring package nor any of your Monitoring sessions may be transferred to another person.

Scheduling Your Monitoring Sessions

All Monitoring sessions are scheduled in advance by direct agreement between both participants and us.

All the Monitoring sessions in your package must be used within 3 months of your order confirmation.  After that time, any unused Monitoring session(s) will be deemed forfeited and shall be forfeited.

Cancellations and Refunds

We do not offer refunds for any of our Monitoring programs or Monitoring sessions.

If You or Your Spouse Are Late for a Scheduled Monitoring Session

If you or your spouse are late for a scheduled session for whatever reason, the session will run to the original agreed time.

If one of you fail to show within 30 minutes of the agreed start time, that person will be considered a “no show” and will forfeit his/her right to the session even if it is still ongoing for the other person.  Such “no show” sessions will not be rescheduled.

If You and Your Spouse Cancel a Scheduled Monitoring Session

If for any reason both you and your spouse cannot attend a scheduled Monitoring session, you are responsible for notifying us by phone or email and ensuring that we receive your communication.  The following apply:

  • Cancellations received by us at least 48 hours in advance of a scheduled session will be rescheduled with you both.
  • Cancellations received by us less than 48 hours in advance of a scheduled session will not be rescheduled, and the session time will be deemed forfeited by you both.

If Epanafero Ltd. Cancels a Scheduled Monitoring Session

On rare occasions, due to unforeseen circumstances, we may need to cancel a scheduled session.  We will notify you of such a cancellation with as much notice as possible and will reschedule with you both.

5. Wellness Retreats

5.1 Wellness Residential Retreat

Please note that our Wellness Residential Retreats are those which are 2-3 days in duration and involve you staying overnight at our venue of choice.

Payment

If you book more than 30 days before the start date of your Wellness Residential Retreat: An advance deposit of 50% of the amount due with respect to your retreat package is required at the time of booking to guarantee your reservation. The full balance is due 30 days prior to start date of your retreat.

If you book 30 or fewer days before the start date of your Wellness Residential Retreat: The full amount of your retreat is required at the time of booking.

Your Wellness Residential Retreat may not be transferred to another person.

Late Arrivals / Early Departures

There are no adjustments or refunds extended if you arrive late or depart early.

Cancellations and Refunds

If You Cancel Your Wellness Retreat

  • If you cancel more than 30 days prior to start date of your Wellness Residential Retreat: A cancellation fee (USD 200 per person) will be applied, and the balance which remains from the amount you have paid will be refunded to you OR held on account (at your election) for ONE (1) YEAR following the date of cancellation for your use towards a future retreat.
  • If you cancel between 14-30 days prior to start date of your Wellness Residential Retreat: A cancellation fee (USD 200 per person) will be applied, your advanced deposit will be forfeited, and the balance which remains from the amount you have paid will be held on account for ONE (1) YEAR following the date of cancellation for your use towards a future retreat.
  • If you cancel fewer than 14 days prior to start date of your Wellness Residential Retreat: The full amount you have paid will be forfeited.

No refund is available if you do not show up for your retreat.

No refund is available if you attend a retreat and are dissatisfied with its presentation or content.

Any monies held on account which are not used by you within the time frame specified above will be forfeited.

If Epanafero Ltd. Cancels Your Wellness Retreat

On rare occasions, due to unforeseen circumstances, we may need to cancel a wellness retreat.  We will notify our participants of such a cancellation a minimum of SEVEN (7) calendar days in advance of the retreat’s start date and will offer a full refund for the retreat OR the option to rebook another retreat within ONE (1) YEAR of the cancellation notice.

If you are purchasing an airfare to attend any of our Wellness Retreats, we recommend that you consider travel insurance with appropriate coverage for ultimate flexibility as we will not reimburse any of your travel expenses.

5.2 Wellness Day Retreat

Please note that our Wellness Day Retreats are those which are one day or less in duration, and do not involve you staying overnight at our venue of choice.

Payment

Payment in full must be made in advance for all Wellness Day Retreats.

Your Wellness Day Retreat may not be transferred to another person.

Late Arrivals / Early Departures

There are no adjustments or refunds extended if you arrive late or depart early.

Cancellations and Refunds

If Epanafero Ltd. Cancels Your Wellness Retreat

6. Online Cardiology Consultation

Payment

Payment in full must be made in advance for your Cardiology Consultation.

Your Cardiology Consultation may not be transferred to another person.

Scheduling Your Cardiology Consultation

Your Cardiology Consultation appointment is scheduled in advance by direct agreement between you and us.

Your Cardiology Consultation must be used within 3 months of your order confirmation.  Beyond this time, the obligation to provide the Consultation be deemed forfeited and shall be forfeited.

Cancellations and Refunds

We do not offer refunds for our Cardiology Consultation appointments.

If You Are Late for Your Scheduled Cardiology Consultation

If you are late for a scheduled appointment for whatever reason, the session will run to the original agreed time.  If, however, you fail to show within 15 minutes of the agreed start time, you will be considered a “no show” and you will forfeit your right to the appointment missed.  Such “no show” appointments will not be rescheduled.  Instead, you will be invited to make the necessary payment to book a new Cardiology Consultation appointment.

If You Cancel Your Scheduled Cardiology Consultation

If for any reason you cannot attend your scheduled appointment, you are responsible for notifying us by phone or email and ensuring that we receive your communication.  The following apply:

  • Cancellations received by us at least 48 hours in advance of a scheduled appointment will be rescheduled with you.
  • Cancellations received by us less than 48 hours in advance of a scheduled appointment will not be rescheduled, and the session time will be deemed forfeited and shall be forfeited.

If Epanafero Ltd. Cancels Your Scheduled Cardiology Consultation

On rare occasions, due to unforeseen circumstances, we may need to cancel a scheduled appointment.  We will notify you of such a cancellation with as much notice as possible and will reschedule with you.

7. Online Heart Health Evaluation

Payment

Payment in full must be made in advance for your Heart Health Evaluation.

Your Heart Health Evaluation may not be transferred to another person.

Scheduling Your Heart Health Evaluation

Your Heart Health Evaluation appointment is scheduled in advance by direct agreement between you and us.

8. In-person Cardiology Appointment

Payment

Payment in full must be made immediately after your in-person Cardiology Appointment.

Your Cardiology Appointment is scheduled in advance by direct agreement between you and us.
